29 CFR 1910.95(i)(5): The employer did not ensure proper initial fitting and supervise the correct use of all hearing protectors. Masa Plant - The employer did not ensure that employees were correctly wearing hearing protection. Employees were modifying their Honeywell Maximum Earplug by creating holes in them to accommodate speaker wires which compromised the earplugs effectiveness to provide hearing protection.
