29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a)Menards Inc. Store #3097 at 2100 Ninth Avenue South East, Watertown, SD 57201: On or about and at times prior to December 19, 2022, the employer did not ensure that forklift operators were reevaluated and retrained at least once every three years. This condition exposed employees to struck-by hazards associated with improper forklift operations.
