29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   Location: 174 French Street, Tewksbury, MA;  On or about 12/19/2022, the employer did not ensure that eye protective equipment was used when employees were exposed to eye injury while using pneumatic nail guns.

29 CFR  1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds per employee attached.  Location: 174 French Street, Tewksbury, MA;  On or about 12/19/2022, employees were using two lifelines connected to one anchor, exposing themselves to falls by exceeding the safety factor and manufactures' instructions.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  Location: 174 French Street, Tewksbury, MA;  On or about 12/19/2022, employees who did not have certification of fall protection training were permitted to perform roofing activities greater than 25-feet above a lower level.
