The Warehouse - On or about December 20, 2022, the rack location 7801B002 was spot loaded on the grate and the grate was collapsing in due to the weight, exposing employees to struck by hazards.   e. The Warehouse - On or about December 20, 2022, the rack location 7727B055 was spot loaded on the grate and the grate was bending upward due to the weight, exposing employees to struck by hazards.   f. The Warehouse - On or about December 20, 2022, the rack location 4255B111, the connector was not engaged into the rack system creating an unstable load, exposing employees to struck by hazards.  a. Warehouse - On or about December 20, 2022, employees were exposed to struck by hazards from unstable storage as the pallet of Maples Rugs in location 78-02-B003 was hanging out of the rack system.  b. Bulk Storage - On or about December 20, 2022, the Sterilite pallets were stacked 3 high, the bottom pallet was damaged which caused for the load to be leaning, employees were exposed to a struck by hazard.   c. Bulk Storage - On or about December 20, 2022, the top pallet of the EMP Scandia Oaknord was leaning due to broken pallet and hanging off the lower tiers, employees were exposed to a struck by hazard.   d. Bulk Storage - On or about December 20, 2022, the stacking frame of rugs 419127 was missing a stanchion causing materials to hang out exposing employees to a struck by hazard.   e. Bulk Storage - On or about December 20, 2022, the stacking frames 7149A098, 4734A069, 6548AL95, 6527A053, were not set properly causing the loads to be unstable, employees were exposed to a struck by hazard.  Abatement certification required within 10 days after abatement date.
