29 CFR  1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck.  Employees engaged in material handing activities using the Daewoo forklift, serial number CX-02963, model G25E-3, were exposed to struck-by hazards in that comprehensive forklift training was not provided prior to permitting forklift operation.

29 CFR  1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour  Employees working in the truck bay area were exposed to fire and explosion hazards. The employer did not ensure that oxygen cylinders were stored separately from highly combustible materials. Oxygen cylinders were being stored immediately adjacent to acetylene and propane cylinders.
