29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At the job site at 201 W. Old 75 HWY, Fairview, Kansas, employees engaged in sign removal activities were exposed to fall hazards of approximately 64 feet from the bottom of the sign to the ground below when accessing and working inside a sign box. Employees were first exposed to a fall hazard when they disconnected their fall harness from the telehandler lift anchor points and exited the basket. Employees were exposed a second time to a fall hazard inside the sign box, walking on the aluminum sheet metal floor without their fall protection connected to an anchor.

At the job site at 201 W. Old 75 HWY, Fairview, Kansas, employees engaged in sign removal activities were exposed to permit-required confined space safety hazards because the employer did not ensure a competent person identified all confined spaces in which employees may work. Employees performing signed work were doing so inside the sign box which is by definition a confined space.
