29 CFR  1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  (a) On or before December 21, 2022, the employer failed to ensure that walking-working surfaces were maintained free of leaks and spills. Water was flowing into the walking working area from a leaking pump in the water treatment area. This exposes workers to slip and fall  hazards.

29 CFR  1910.151(c):Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  (a) On or before December 21, 2022 the employer failed to ensure suitable facilities for quick drenching or flushing of the eyes and body within the lime slurry mixing area on the 2nd floor of the water treatment room. This condition exposed the employees to the risk of permanent damage to the eyes.

29 CFR  1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:  (a) On or before December 21, 2022, the employer did not maintain workspace of at least 3 feet in front of (2) 120-volt electrical panes behind the bailer in the warehouse area. One electrical panel extended approximately 6 inches from the wall with a spill kit located 39 inches from the wall. This left a clearance of approximately 32 inches.  The required clearance is 36 inches. This condition exposed employees to electrical and mechanical hazards.

29 CFR  1910.147(d)(4)(ii):Lockout devices utilized were not affixed in a manner that held the energy isolating device in a safe or off position:  (a) On or before  December 27, 2023 the employer failed to ensure lockout devices were affixed in a matter that held the energy isolating device in safe or off position.  The lockout for the Sidel Labeler on bottling line one was missing.  The lockout point was rendered inoperable and exposed employees to injuries associated with the unexpected energizing of equipment including, amputations, burns, struck-by injuries, and other serious injuries.
