29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): a) On or about January 3, 2023, an employee was exposed to live parts of the TYTAN tankless water heater operating at or below 480-volts during a quality control inspection in the loop area; the employer did not guard the employee against accidental contact of an electrical shock hazard of 480-volts or less by guarding the line side of the control switch located inside of the enclosure.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed. a) On or about January 3, 2023, an employee was exposed to a 480-volt electrical shock hazard from unprotected conductors during a quality control inspection of the TYTAN tankless water heater in the loop area; the employer did not ensure that the 3, 1/0 conductors entering a cabinet were protected from abrasion and the opening through which the conductors entered was effectively closed.

29 CFR 1910.305(b)(1)(iii): Where cable is used, each cable shall be secured to the cabinet, cutout box, or meter socket enclosure. a) On or about January 3, 2023 employees were exposed to a 480-volt electrical shock hazard from the cable sheath not continuing into the green enclosure during a quality control inspection of the TYTAN tankless water heater in the loop area. The employer did not ensure that the 3 individual 1/0 conductors exiting the top of the green enclosure and the 3 sheathed 1/0 conductors entering the bottom of the green enclosure were protected from abrasion by providing a fitting on each end.
