29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net system, personal fall arrest system, or a combination of a warning line system and safety monitoring system. On or about January 5, 2023, employees engaged in roofing activities 6 feet or more above lower levers were not provided fall protection. This condition exposed employees to fall hazards above 6 feet high.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill. Construction site: On or about November January 5, 2023, the employer did not ensure that employees working on a commercial flat roof retained the requisite understanding or skill for safe work practice. This condition exposed employees to fall hazards greater than 6-feet to the ground below.
