Employee performing roof work fell approximately 12 feet through skylight from roof to ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about: October 12, 2022 Location: 115 South Orange Ave., West orange, NJ 07052 a) The employer did not ensure that ladders were extended at least 3 feet above the landing surface. Employees accessed a roof approximately 12 feet from the ground below from a fully extended portable ladder whose side rails were not extended over upper landing surface. Green Master Contractor Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard previously 1926.1053(b)(1), which was contained in OSHA inspection number 1601964, citation number 1, item number 4 and was affirmed as a final order on December 28, 2022, with respect to a workplace located at 4 Loganberry Development 1128 Barney Jenkins Road, Felton, DE 19943.
