29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a) Jobsite located at 231 Shurs Lane, Philadelphia, PA 19128: On or about January 13, 2023, the employer did not ensure that employees utilized protective helmets at the jobsite while performing framing operations. Employees were working from the ground level while transporting materials to the roof level (4th floor), thus exposing the employees to overhead struck-by hazards.

29 CFR  1926.302(b)(6): The use of hoses for hoisting or lowering tools shall not be permitted:   a) Jobsite located at 231 Shurs Lane, Philadelphia, PA 19128 - On or about January 13, 2023, the employer did not prohibit the use of hoses for hoisting and/or lowering tools. Employees were adhering insulation panels to a residential structure by lowering a pneumatic nail gun by its hoses from the roof level and swinging it against the siding to activate contact-trip trigger, thereby exposing employees to struck-by hazards.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a) Jobsite located at 231 Shurs Lane, Philadelphia, PA 19128: On or about January 13, 2023, the employer did not ensure employees engaged in residential construction activities 6 feet or more above lower levels were protected by guardrail systems, safety net system, or personal fall arrest system. Employees were framing the fourth story of a residential building without the use of fall protection, thereby exposing employees to fall hazards of approximately 30 feet.

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) Jobsite located at 231 Shurs Lane, Philadelphia, PA 19128 - On or about January 13, 2023, the employer did not ensure eye protection was used by employees engaged in residential construction activities. Employees engaged in residential framing activities used pneumatic nail guns without the use of eye protection, exposing employees to flying particles.  Green Master was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1600152, citation number 1, item number 1 and was affirmed as a final order on July 20, 2022, with respect to a workplace located at 460 Route 22, Whitehouse Station, NJ.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Jobsite located at 231 Shurs Lane, Philadelphia, PA 19128 - On or about January 13, 2023, the employer did not provide a training program for each employee conducting residential framing activities in the recognition and avoidance of fall hazards. Employees were framing the fourth story of a residential building in close proximity to unprotected sides and edges without fall protection, thereby exposing employees to falls of approximately 30 feet.  Green Master was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(2), which was contained in OSHA inspection number 1589276, citation number 1, item number 4, and was affirmed as a final order on September 21, 2022, with respect to a workplace located at Foundry Boulevard, Mount Holly, NJ 08060.  Green Master was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1), which was contained in OSHA inspection number 1605751, citation number 1, item number 1-b, and was affirmed as a final order on January 4, 2023, with respect to a workplace located at 2216 N Marshall St, Philadelphia, PA 19133.
