29 CFR  1926.503(a)(1):The employer did not train each employee who might be exposed to fall hazards to enable them to recognize fall hazards and the procedures to be followed in order to minimize these hazards:  a) Jobsite at 146 E Nields Street, West Chester, PA - On or about January 9, 2023, the employer did not provide a training program for each employee conducting residential framing activities in the recognition and avoidance of fall hazards. Employees were conducting roofing activities on a three story residential building in close proximity to unprotected sides and edges without the use of fall protection, thus exposing employees to falls ranging from approximately 20 to 30 feet.

29 CFR  1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  a) Jobsite at 146 E Nields Street, West Chester, PA- On or about January 9, 2023, the employer did not ensure that employees were provided training on the recognition and mitigation of hazards associated with the use of ladders. Employees utilized a self-supporting ladder in a closed position, extension ladders did not extend three feet above an upper landing surface, and ladders were used at an angle greater than a 4:1 ratio during roofing operations, thereby exposing employees to fall hazards.

Employees utilized a portable ladder to access the roof of the second and third story without having side rails extended at least 3 feet above the landing surface, thereby exposing employees to fall hazards ranging  approximately 20 to 30 feet.
