29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: (a) In or around December 2022, employees of Heidelberg Materials US Cement, LLC. were exposed to a fall hazard of approximately 11.5 feet when utilizing a metal rod, as a reach tool, to align the discharge spout from an open window located in the second-floor silo control room at: Heidelberg Materials US Cement LLC 8282 Middlebranch Ave. NE, Middlebranch, Ohio.
