Employees conducting maintenance operations on the Molten Zinc Pot Mill No. 6, were exposed to injuries from stepping into holes measuring approximately ten (10) inches by eighteen (18) inches.

29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems.   a) ?On or?about January 3, 2023, the employer did not ensure that a hole measuring approximately ten (10) inches by eighteen (18) inches, on the walking surface of Molten Zinc Pot Mill #6 was provided with a cover. Thereby exposing employees to hazards associated with burns.
