29 CFR 1910.106(e)(6)(i):"General." Adequate precautions shall be taken to prevent the ignition of flammable vapors. Sources of ignition include but are not limited to open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat. a) Wick Processing Department: On or about January 11, 2023 and at times prior, the employer exposed employees to fire hazards, in that flammable chemicals such as but not limited to Acetone and Isopropyl Alcohol were stored in cabinets within 3 to 8 feet of an ignition source from the Flux Machine operating above 260 degrees Fahrenheit while employees are carrying out their daily assigned tasks.

29 CFR 1910.95(d)(1):When information indicates that any employee's exposure may equal or exceed an 8-hour time-weighted average of 85 decibels, the employer shall develop and implement a monitoring program. a) ITW-CCE: On or about January 11, 2023 and at times prior, the employer exposed employees to noise hazards, in that a noise monitoring program was not implemented while employees were performing their duties around wick winding machines, in the Braiding Department, and on the liquid and aerosol lines.
