29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice:  Walk-in freezer:  Ice was forming on the floor at the entrance of the freezer and on the floor beneath cooling unit UC-5. Employees were exposed to slip and fall hazards.  Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.22(a)(3), which was contained in OSHA inspection number 1435615, citation number 1, item number 1 and was affirmed as a final order on 11-06-2019, with respect to a workplace located at 4725 Bougainville Dr. Pearl Harbor, HI 96860.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  Meat department storeroom: A Hamilton Beach Microwave was plugged into a multi-outlet extension cord. Employees were exposed to fire and electrical hazards.  The Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 1398739, citation number 2, item number 2 and was affirmed as a final order on 11/12/2019, with respect to a workplace located at Bldg 6088 Mokapu Road, MCBH Kaneohe Bay HI 96863.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  a. Meat storage chill box: Multiple extension cords were zip tied to insulated pipes to supply power to the cooling units. Employees were exposed to fire and electrical hazards.   b. Outside of the Freezer: An Extension cord was run behind racking systems and up over pipes in the ceiling suppling power to a fan on top of the freezer. Employees were exposed to fire and electrical hazards.  c. Left of grocery bailer: An extension cord was run up the wall zip tied to conduit supplying power to a wall mounted fan. Employees were exposed to fire and electrical hazards.  d. Temporary storage tent: An extension cord was run from the warehouse outside to a temporary storage tent supplying power to a light string. Employees were exposed to fire and electrical hazards.  The Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.305(g)(1)(iv)(A), which was contained in OSHA inspection number 1398739, citation number 2, item number 3 and was affirmed as a final order on 11/12/2019, with respect to a workplace located at Bldg 6088 Mokapu Road, MCBH Kaneohe Bay HI 96863.    The Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(g)(1)(iv)(A), which was contained in OSHA inspection number 1314559, Notice Number 1, Item Number 1, and was affirmed as a final order on 06/14/2018, with respect to a workplace located at Bldg. 795 Reeves Blvd., Lemoore Naval Air Station, CA 93246.
