29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) Third Floor Finishing Department: On January 5, 2023, employees were exposed to machine hazards associated with moving parts and in-going nip points when servicing and adjusting of brush rollers, on the Brush Finishing Machine BFM2 . The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5), and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. (b)Third Floor Finishing Department: On or about January 11, 2023, employees were exposed to machine hazards associated with thermal and crushing hazards during servicing and cleaning of the Krause #7283 football embossing press plate. The employer failed to implement energy control application steps as LOTO devices were not applied to the electrical disconnect [per the 1910.147(d)(4) requirements]. As a result, the remaining applicable energy control elements, involving dissipation of residual energy [(d)(5), and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

(b)Third Floor Finishing Department: On March 7, 2023, the employer did not ensure guarding was equipped at the point of operation on the Brush Feeding. Employees were exposed to pull in hazards and rotating parts when hand feeding material at the front of the machine. (c)Third Floor Finishing Department: On March 7, 2023, the employer did not ensure that guarding was affixed on the Small Spray machine at the spray coating section. Employees were exposed to struck by hazards from the rotating spray heads.
