On or about January 12, 2023, in the preparation hangar, employees performing prep work used ladders with defects such as, but not limited to broken feet, broken spreaders, illegible labels due to overspray, and steps covered in overspray that were not removed from service.

(a)  On or about January 12, 2023, in the preparation hangar, employees performing prep work used A-frame ladders that were covered in overspray such that the steps were no longer corrugated.  (b) On or about January 12, 2023, in the preparation hangar, employees performing prep work used an A-frame ladder that had the bottom step repaired with sheet metal and the corrugation was not maintained.

Rose Aircraft Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1550437, citation number 1, item number 2b and was affirmed as a final order on April 18, 2022, with respect to a workplace located at 132 Flight Lane, Mena, AR 71953.
