29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  (a) On or about January 10, 2023, the employer who was the competent person did not ensure that frequent and regular inspections of the job site were conducted to identify hazards and the job site conditions were maintained safe.

29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) On or about January 10, 2023, employees working on the ground to perform cleanup and provide assistance to the roofers who were working directly above re-roofing the structure, were not protected from struck by hazards from falling materials and tools by wearing a protective helmet/ hard hat.

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On, or about January 10, 2023 employees operating pneumatic nail guns without eye or face protection while installing shingles, were exposed to struck by hazards that cause puncture wounds and potential blindness or eye injury.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b)  (a) On, or about January 10, 2023, employees conducting roofing work on an 6:12 pitch roof on the South and East side of the structure were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately nine (9) foot eight (8) inch to Fifteen Feet.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) On, or about January 10, 2023, the employer did not insure employees had received  fall protection training for employee(s) exposed to fall hazards while working on a 6:12 pitch residential roof without a fall protection system installed, to recognize hazards associated with this fall hazard, and to give employees the knowledge needed to reduce, or remove the hazard.
