(a) On or about June 1, 2023, the employer who was the competent person did not ensure that frequent and regular inspections of the job site were conducted to identify hazards and the job sites were maintained safe. The JC Siding LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 1643732, citation number 1, item number 1 and was affirmed as a final order on 3/24/2023, with respect to a workplace located at 1825 Yorkville Meadows Union Grove, WI 53182.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b) (a) On or about June 1, 2023, employees conducting roofing work on an 6:12 pitch roof on the south and east side of the structure were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 11 feet 5 inches on the east side of the roof, and to 15 feet 8 inches on the south side of the roof. The JC Siding LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1643732, citation number 1, item number 3 and was affirmed as a final order on 3/24/2023, with respect to a workplace located at 1825 Yorkville Meadows, Union Grove, WI 53182. The JC Siding LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1579161 , citation number 1, item number 1 and was affirmed as a final order on 7/1/2022 , with respect to a workplace located at 1964 Cheyenne Avenue, Grafton, WI 53024.

29 CFR 1926.1053(b)(1): Where a portable ladder(s) was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. (a) On or about June 1, 2023, the Werner Extension ladder erected on the East side of the structure, at the front entrance, being used for access to the 6:12 pitched roof, did not extend at least three (3) feet above the landing edge, and/or was not secured to the structure when it was not sufficiently extended. This exposed employee accessing the roof to a fall hazard of approximately 11 feet, 2 inches.
