A power-ventilated structure provided to enclose or accommodate a spraying operation to confine and limit the escape of spray, vapor, and residue, and to safely conduct or direct them to an exhaust system:  a) C & T Custom Fabrication, 345 Basher Dr., Berthoud, CO 80513: On or about January 03, 2023, and at times prior,  employees were exposed to inhalation and dermal hazards from over spraying of powder coat on larger metal pieces outside of booth.

29 CFR  1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  a) C & T Custom Fabrication, 345 Basher Dr., Berthoud, CO 80513: On or about January 03, 2023, and at time's prior, employees were exposed to respiratory hazards from welding and powder coating spray operation. The employer did not provide medical evaluations to employees required to use the respirators as required in 1910.134(c)(1)(ii).  b) C & T Custom Fabrication,  345 Basher Dr., Berthoud, CO 80513: On or about January 03, 2023, and at time's prior, employees were exposed to respiratory hazards from welding and powder coating spray operation. The employer did not provide fit testing procedures for tight-fitting respirators to employees required to use the respirators as required in 1910.134(c)(1)(iii).  c) C & T Custom Fabrication, , 345 Basher Dr., Berthoud, CO 80513: On or about January 03, 2023, and at time's prior,  employees were exposed to respiratory hazards from welding and powder coating spray operation. The employer did not provide procedures for proper use of respirators in routine and reasonably foreseeable emergency situations for tight-fitting respirators to employees required to use the respirators as required in 1910.134(c)(1)(iv).  d) C & T Custom Fabrication, , 345 Basher Dr., Berthoud, CO 80513: On or about January 03, 2023, and at time's prior,  employees were exposed to respiratory hazards from welding and powder coating spray operation. The employer did not provide procedures for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators of employees required to use the respirators as required in 1910.134(c)(1)(v).  e) C & T Custom Fabrication, 345 Basher Dr., Berthoud, CO 80513: On or about January 03, 2023, and at time's prior,  employees were exposed to respiratory hazards from welding and powder coating spray operation. The employer did not provide training of employees in the respiratory hazards to which they are potentially exposed during routine and emergency situations for employees required to use the respirators as required in 1910.134(c)(1)(vii).  f) C & T Custom Fabrication, , 345 Basher Dr., Berthoud, CO 80513: On or about January 03, 2023, and at time's prior,  employees were exposed to respiratory hazards from welding and powder coating spray operation. The employer did not provide training on putting on, removing, or the limitations of the respirators for employees required to use the respirators as required in 1910.134(c)(1)(viii).  g) C & T Custom Fabrication, , 345 Basher Dr., Berthoud, CO 80513: On or about January 03, 2023, and at time's prior,  employees were exposed to respiratory hazards from welding and powder coating spray operation. The employer did not establish work-site specific procedures to regularly evaluate the effectiveness of the program as required in 1910.134(c)(1)(ix).

29 CFR  1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:   a) C & T Custom Fabrication, 345 Basher Dr., Berthoud, CO 80513: On or about January 03, 2023, and at times prior, employees were exposed to chemical hazards while working with chemicals such as but not limited, to; 1, 3, 5-Triglycidyl Isocyanurate (Industrial Powder Coating). The employer failed to provide the required safety data sheet for each hazardous chemical in use.     b) C & T Custom Fabrication, 345 Basher Dr., Berthoud, CO 80513: On or about January 03, 2023, and at times prior, employees were exposed to chemical hazards while working with chemicals such as but not limited, to; 1, 3, 5-Triglycidyl Isocyanurate (Industrial Powder Coating). The employer failed to provide effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.
