29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) New Horizons Baking Co. On or about January 13, 2023, at the location of 211 Woodlawn Ave. Norwalk, OH: The employer did not ensure the in going nip point created between the V belt conveyors and pulleys on the "Bun Line" were adequately guarded. Employees assisting movement of baking pans in proximity of this condition were exposed to amputation hazards.

The descriptions in Column F lacked the required information, including, but not limited to the object or task that caused the injury and the part of the body affected.
