29 CFR 1910.147(c)(7)(i):The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. a). New Horizons Baking Co. On or about July 28, 2023, at the location of 211 Woodlawn Ave. Norwalk, OH: Employer did not adequately train employees to ensure that the purpose and function of the energy control program was understood by employees. Employees replacing the LeMatic Inc. BF-25W Band Slicer blade without the benefit of recognizing and controlling potentially hazardous energy were exposed to caught in hazards.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in 29 CFR 1910.147(d)(1)-(6) and was not done in sequence as required by 29 CFR 1910.147(d). a). New Horizons Baking Co. On or about July 28, 2023, at the location of 211 Woodlawn Ave. Norwalk, OH: The employer failed to ensure established procedures for the application of energy control was done in sequence to ensure the LeMatic Inc. BF-25W Band Slicer is fully deenergized prior to the servicing/maintenance task. The employees were replacing the Band Slicer blade without the benefit of fully controlling all potentially hazardous energy. Due to this condition, the employee was exposed to a caught in hazard.

New Horizons Baking Co. On or about July 28, 2023, at the location of 211 Woodlawn Ave. Norwalk, OH: Employer did not adequately guard the grouper bun break and gate on line 1, 2, and 3 of the bun line to ensure employees were protected from the moving equipment. Employees working within proximity of the actuating bun gate without the benefit of guarding, were exposed to a crushing hazard.
