29 CFR  1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a)Production Line 3000: On or about January 17, 2023, and times prior, employees performed servicing, such as adjusting or gauging the roller height on production line 3000.  The employer did not develop or utilize adequate energy control procedures that would protect employees from ingoing nip points while adjusting the roller heights.     In the Alternative:  29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Production Line 3000: On or about January 17, 2023, and times prior, employees adjusted or gauged the roller height on production line 3000 and the rollers were not adequately guarded to prevent employees from being pulled into the machine.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) 395 Steeley Way Kearneysville, WV 25430: On or about December 22, 2022, the employer was made aware of a workplace accident that resulted in an inpatient hospitalization.  OSHA was not notified of the hospitalization until January 17, 2023.
