29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. a) L3000; On or about August 3, 2024 employees locked out only the pneumatics of the machine to perform a spindle change out. A lockout procedure was not developed that addressed all of the energy sources, such as 480 electrical, hydraulic, and kinetic exposing employees to caught-in and crushing injuries. b) L2000; On or about August 1, 2024 two production employees were removing material from rollers. The employees bypassed machine guards and did not lock out the machine exposing the employees to caught-in and crushing Injuries.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) L3000; On or about August 3, 2024 and times prior, employees performed tasks such as replacing spindles, that required the use of LOTO. The employees were not trained as authorized or affected employees, exposing them to hazards such as crushing, struck by, caught in and amputations. b) L2000; On or about August 1, 2024 two production employees performed tasks such as cleaning and removing material from rollers that required the use of LOTO. The employees were not trained as authorized or affected employees and did not recognize hazards such as caught-in, struck by and electrical from failure to LOTO.
