29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At a worksite located at the corner of Cedar Crest Drive and Copperleaf Street, Raymore Mo, 64083, an employee was exposed to a fall hazard due to the improper use of a portable extension ladder. The employer exposed the employee to this hazard by allowing them to use a portable extension ladder with side rails that were not extended at least three feet above the surface to which the ladder was being used to gain access.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): At a worksite located at the corner of Cedar Crest Drive and Copperleaf Street in Raymore, Missouri 64083, an employer exposed an employee to fall hazards greater than six feet. The employee was exposed to these hazards as the employer allowed them to work at heights greater than six feet without the use of appropriate fall protection. Imperial Drywall LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1411481, citation number 1, item number 1, and was affirmed as a final order on 11/19/2019, with respect to a workplace located at 6743 W 135th Street, Overland Park, KS, 66223.
