29 CFR 1910.132(d)(2):The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. On or about January 18, 2023, at a residence located at 1880 Stillman Drive Oshkosh, Wisconsin; the employer could not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. Employees eyes and body may be exposed to injurious corrosive materials while cleaning up chemical spills.

29 CFR 1910.305(b)(2)(i):All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. a. On or about January 18, 2023, at the establishment located at 1880 Stillman Drive Oshkosh, WI; employees were exposed to electrical hazards from an outlet box missing a cover. b. On or about January 18, 2023, at the establishment located at 1880 Stillman Drive Oshkosh, WI; employees were exposed to electrical hazards from a double gang junction box with a faceplate that had one opening.
