29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: a) Warehouse - On May 8, 2024, the employer did not ensure that the Toyota forklift model #8FGCU25, serial #88086, powered industrial truck(s) with the damaged back tires, and missing pedal covers, was withdrawn from service until restored to safe operating condition. b) Warehouse - On May 8, 2024, the employer did not ensure that the Mitsubishi, model #FGC25N, serial #AF82F49490, powered industrial truck(s) with a broken mirror, was withdrawn from service until restored to safe operating condition. c) Warehouse - On May 8, 2024, the employer did not ensure that the Toyota forklift model #8FGCU25, serial #44101, powered industrial truck(s) with the missing pedal covers, was withdrawn from service until restored to safe operating condition. d) Warehouse - On May 8, 2024, the employer did not ensure that the UniCarrier forklift model #MCP1F2A25LV, serial #CP1F2-9W37218, powered industrial truck(s) with the damaged back left tire, and broken front side mirror, was withdrawn from service until restored to safe operating condition. e) Warehouse - On May 8, 2024, the employer did not ensure that the Toyota forklift model #8FGCU25, serial #84340, powered industrial truck(s) with no side mirror, was withdrawn from service until restored to safe operating condition. f) Warehouse - On May 8, 2024, the employer did not ensure that the Toyota forklift model #8FGCU25, serial #77896, powered industrial truck(s) with the damaged back tires, and no pedal covers, was withdrawn from service until restored to safe operating condition. g) Warehouse - On May 8, 2024, the employer did not ensure that the Toyota forklift model #8FGCU25, serial #88147, powered industrial truck(s) with the damaged to the tip of the left fork, was withdrawn from service until restored to safe operating condition.
