Employees working in this condition were exposed to potential struck by hazards (flying projectile/ricochet nails).

On or about January 18, 2023 at the address of 29737 River Rd. Perrysburg OH 43551: The employer did not ensure employees were protected from falls while performing roofing work activities atop a two-story residential structure. Employees operating in this condition were exposed to falls of 25ft to the ground below. Roofing Services Plus Inc was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1413106, citation number 1, item number 2 and was affirmed as a final order on August 8, 2019, with respect to a workplace located at 4239 Holt Rd. Sylvania OH, 43560.
