29 CFR  1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) Roofing Services Plus Inc. at the worksite located at 2942 Riva Ridge Road, Ottawa Hills, Ohio, 43615:  On or about August 27, 2025, the employer did not have an accident prevention plan or a program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury or illness.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   a) Roofing Services Plus Inc. at the worksite located at 2942 Riva Ridge Road, Ottawa Hills, Ohio, 43615: On or about August 27, 2025, the employer did not ensure that employees had been trained in fall hazard awareness or appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while performing various roofing operations from elevated surfaces approximately 12 feet or greater to the ground below.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Roofing Services Plus Inc. at the worksite located at 2942 Riva Ridge Road, Ottawa Hills, Ohio, 43615:  On or about August 27, 2025, the employer did not ensure that the portable ladders used by employees to access the second level of a residential home to perform roofing activities were extended 3 feet above the landing surface of the roof, exposing employees to fall hazards of approximately 12 feet or greater to the ground below.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Roofing Services Plus Inc. at the worksite located at 2942 Riva Ridge Road, Ottawa Hills, Ohio, 43615:  On or about August 27, 2025, the employer did not ensure employees were protected from falls while performing residential roofing activities atop a 10:12 pitch residential roof.  Five employees were exposed to falls of approximately 12 feet or greater to the ground below.  Roofing Services Plus Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA Inspection number 1644394, citation number 2, item number 1 and was affirmed as a final order on March 16, 2023, with respect to workplace located at 29737 River Road, Perrysburg, Ohio, 43551.
