On or about January 18, 2023, facility wide, the employer did not ensure that employee(s) who operate powered industrial vehicles, including the Yale electric forklift, Model ERP040VTN36TE082, Serial Number G807N14940U, were evaluated as part of training to ensure their competence to perform tasks in a safe manner, thereby exposing employees to struck-by hazards.
