29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  (a ) Grinding Room: On January 11, 2023, employees were exposed to machine hazards associated with moving parts when performing service and maintenance such as but not limited to the cleaning of the small Provisur Meat Grinder meat grinding cutting plates with openings larger enough to allow an employees finger to slip through and make contact with the cutting knife which is located directly behind the plate. The employer failed to implement energy control application steps such as LOTO devices were not applied to the electrical disconnect was not performed [per the 1910.147(d)(4) requirements]. As a result, the remaining applicable energy control elements, involving dissipation machine isolation [(d)(5)(2)], and verification of isolations [(d)(6)], were not  implemented to protect employees from machine servicing hazards.   (b) Facility Wide : On or about January 19, 2023, employees were exposed to machine hazards associated with moving parts when performing service and maintenance such as but not limited to the changing of meat grinder plates on the Large and Small Grinders. The employer failed to implement energy control application steps such as LOTO devices were not applied to the electrical disconnect was not performed [per the 1910.147(d)(4) requirements]. As a result, the remaining applicable energy control elements, involving dissipation machine isolation [(d)(5)(2)], and verification of isolations [(d)(6)], were not  implemented to protect employees from machine servicing hazards. 29 CFR 1910.147(f)(3)(i): When servicing and/or maintenance is performed by a crew, craft, department or other group, they shall utilize a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device.  (a) Facility Wide : On or about January 19, 2023, the employer did not ensure when service and maintenance such as but not limited to the disassembly and sanitation of equipment such as but not limited to grinders, mixers, stuffers, patty machines, pork sausage link machines by a group of employees,  that a procedure was utilized that would afford each employee a level of protection equivalent to that provided by the implementation of a personal lockout/tagout device.
