29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: Construction site: On or about January 20, 2023, an employee was observed in an excavation (11feet 8inches) deep without a safe means of egress. An employee was working in one trench box while the ladder was located in another trench box next to the one the employee was working in. This condition exposed the employee to cave-in hazards.

29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: Construction site: On or about January 20, 2023, an employee was not provided with adequate protection from loose rock or soil from an excavation. Employee was working in between two trench boxes (4 feet 4 inches apart) with loose soil and large rock at the edge of the excavation, directly above the employee below. This exposed employee to struck-by hazards.
