29 CFR 1910.179(l)(1): Preventive maintenance. A preventive maintenance program based on the crane manufacturer's recommendations shall be established. On or about January 10, 2023, at an establishment located at 200 Main Avenue in Green Bay, Wisconsin; at the Super Calendar #3, the employer did not develop a preventive maintenance program based on the crane manufacturer's recommendation of a General Overhaul once the Safe Working Period had reached zero exposing employees to potential struck by hazards.

29 CFR 1910.179(j)(3)(ii): Any deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard: Loose bolts or rivets. On or about January 10, 2023, at an establishment located at 200 Main Avenue in Green Bay, Wisconsin; at the Super Calendar #3, the periodic inspection of the R & M hoist assembly attached to the Contrx Crane trolley system did not include an inspection of all fasteners to ensure secured exposing employees to potential struck by hazards.
