29 CFR  1926.452(m)(1):Crawling boards shall extend from the roof peak to the eaves when used in connection with roof construction, repair, or maintenance.  Jobsite - On or about January 11, 2023, an employee performing roofing work used a crawling board on the roof that did not extend from the roof peak to the eave. The approximately 8-foot section was located on the lower portion of the roof and did not extend to the roof peak.

Jobsite - On or about January 11, 2023, an employee used a crawling board on the roof that was not secured to the roof by ridge hooks or other adequate means. The approximately 8-foot section was secured at the base by the ladder used for access to the roof.

Site - On or about October 11, 2023, the extension ladder utilized for roof access was not adequately installed in that the ladder did not extend 3 feet above the upper landing surface of the roof.
