29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: On or about 1/23/2023, inspections were not performed to identify and eliminate the following hazardous conditions of the excavation: a) The manhole excavation had vertical walls approximately 13.5 feet tall; b) The ladder was staged in an unprotected area of the trench; c) The trench, at the ladder, had vertical walls approximately 6.5 feet tall; d) The bottom of the trench box was approximately 3.5 feet above the floor of the excavation; e) The travel distance to the ladder was approximately 36 feet from the near side of the manhole and 41 feet from the far side of the manhole.
