In the warehouse loading and unloading area a relocatable power tap was used in palace of permanent wiring to power a product scale. The condition exposed employees to hazards of burns, fire, and electrical shocks.

29 CFR 1910.30(a)(3)(iv): The employer did not train each employee in the correct use of personal fall protection systems and equipment specified in paragraph (a)(1) of this section, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer. (a) On or before 1/24/2023 the employer failed to train each employee on the correct use of equipment. The employer provided training did not include use of the "Manual Lowering Valve" to rescue employees that are unable to reach the controls after a fall and suspension on safety belt.
