A) Consolidated Edison Co. - Holsman Rd. and Campus Rd., Staten Island NY 10301 - The employer did not ensure the electrical lines and equipment were considered and treated as energized while employees were moving 4KV cables to new utility poles. Employees were exposed to the hazard of electrical shock and burns. Condition occurred on January 18, 2023.

29 CFR 1910.269(m)(3)(i): The employee that the employer designates pursuant to paragraph (m)(2) of this section as being in charge of the clearance shall make a request of the system operator to deenergize the particular section of line or equipment. The designated employee becomes the employee in charge (as this term is used in paragraph (m)(3) of this section) and is responsible for the clearance. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer failed to report the in-patient hospitalization, amputation, or loss of an eye to OSHA. A worker sustained injuries due to an electrical shock incident that occurred on January 18, 2023 and was hospitalized the same day.
