29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: a) Northwest corner: On January 20, 2023, and times prior employees who performed stucco activities from a framed scaffold were not provided with a safe means of access. Employees climbed the scaffold frames, climbed out from balconies, or over a deck wall to access their work areas. This exposed the workers to fall hazards up to 48-feet from the top level of the scaffold down to the ground below. Note) Certification and documentation of corrective actions are required for this item.

The scaffold was five (5) feet below the top of the wall and up to 21-inches away from the face of the building. This exposed the workers to fall hazards up to 48-feet from the top of the parapet wall down to the ground below.
