On an aerial lift at a commercial business, at a temporary worksite located at 1331 University Boulevard, in Jacksonville, FL: On or about January 25, 2023, Majestic Eagle Construction, failed to ensure that its subcontractor Bautista O. Construction protected its employees from fall hazards, in that fall protection was not utilized by employees while operating an aerial lift performing framing work.
