29 CFR  1910.147(c)(4)(i):  Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.   Note: Exception: The employer need not document the required procedure for a particular machine or equipment, when all of the following elements exist: (1) The machine or equipment has no potential for stored or residual energy or reaccumulation of stored energy after shut down which could endanger employees; (2) the machine or equipment has a single energy source which can be readily identified and isolated; (3) the isolation and locking out of that energy source will completely deenergize and deactivate the machine or equipment; (4) the machine or equipment is isolated from that energy source and locked out during servicing or maintenance; (5) a single lockout device will achieve a locker-out condition; (6) the lockout device is under the exclusive control of the authorized employee performing the servicing or maintenance; (7) the servicing or maintenance does not create hazards for other employees; and (8) the employer, in utilizing this exception, has had no accidents involving the unexpected activation or reenergization of the machine or equipment during servicing or maintenance.  (a)  On or about January 25, 2023, at an establishment located at 8404 Venture Circle, in Schofield, Wisconsin, in the Core Press Brake Department, energy control procedures were not developed, documented, or utilized when performing set up activities to address the electrical, pneumatic, hydraulic, and gravity energy sources on the Trumpf TruBend 5230 hydraulic press brake exposing employees to struck by hazards.  (b)   On or about January 25, 2023, at an establishment located at 8404 Venture Circle, in Schofield, Wisconsin, in the Core Press Brake Department, energy control procedures were not developed, documented, or utilized when performing set up activities to address the electrical, pneumatic, hydraulic, and gravity energy sources on the Trumpf TruBend 5085 hydraulic press brake exposing employees to struck by hazards.  (c)  On or about January 25, 2023, at an establishment located at 8404 Venture Circle, in Schofield, Wisconsin, in the Core Press Brake Department, energy control procedures were not developed, documented, or utilized when performing set up activities to address the electrical, pneumatic, hydraulic, and gravity energy sources on the Trumpf TruBend 5130 hydraulic press brake exposing employees to struck by hazards.  (d)   On or about January 25, 2023, at an establishment located at 8404 Venture Circle, in Schofield, Wisconsin, in the Panel Department, energy control procedures were not developed, documented, or utilized when performing set up activities to address the electrical, pneumatic, hydraulic, and gravity energy sources on the Bystronics Xpert40, Asset #10083 exposing employees to struck by hazards.  (e)  On or about January 25, 2023, at an establishment located at 8404 Venture Circle, in Schofield, Wisconsin, in the Panel Department, energy control procedures were not developed, documented, or utilized when performing set up activities to address the electrical, pneumatic, hydraulic, and gravity energy sources on the Cincinnati hydraulic press brake exposing employees to struck by hazards.

29 CFR  1910.147(c)(6)(i):  The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  An establishment located at 8404 Venture Circle, in Schofield, Wisconsin, the employer did not conduct a periodic inspection of the developed and utilized energy control procedures at least annually to ensure the procedures were accurate and employee(s) involved knew their responsibilities under the procedure exposing employees to inadvertent/accidental restart of equipment.

Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.  On or about January 25, 2023, at an establishment located at 8404 Venture Circle, in Schofield, Wisconsin, a Toyota #24955 forklift with fork extensions which affected the capacity and safe operations did not have the capacity and operation plate updated to adjust the lifting capacity exposing employees to tip over hazards.

On or about January 25, 2023, at an establishment located at 8404 Venture Circle, in Schofield, Wisconsin, in the Core Press Brake Department; employees were standing next to and holding the diamond plate while a Toyota forklift was used  as a lift assist for the Trumpf 5230 hydraulic press brake when bending exposing employees to struck by hazards.

On or about January 25, 2023, at an establishment located at 8404 Venture Circle, in Schofield, Wisconsin, in the Core Press Brake Department; the Toyota forklift was used  as a lift assist for the Trumpf 5230 hydraulic press brake when bending diamond plate sheets.  The front portion  and tips of the forks were assist lifting the part which was being shaped and then brought back down exposing employees to struck by hazards.

29 CFR  1910.212(a)(1):  Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  On or about January 25, 2023, at an establishment located at 8404 Venture Circle, in Schofield, Wisconsin, in the Panel Department, the back door for the Salvagnini 14 bend press, Serial Number 1201235, was bypassed with another interlock key actuator exposing employees to moving parts when removing parts which were stuck.
