29 CFR 1910.22(a)(2): The employer did not ensure that the floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. The employer did not ensure that when wet processes are used, drainage is be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided: On January 26, 2023, in the work area located at the freezers, employees walking and working in the area were exposed to slip and fall hazards where towels were used to control water accumulating on the floor.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling, in that employees are exposed to electrical shock, fire, or burns when using appliances in the work and break areas. On January 26, 2023, employees were using appliances that were receiving power from a relocatable power tap including, but not limited to the following: a) In the break area - Refrigerator, microwave, and Toastmaster Air Fryer were plugged into the relocatable power tap instead of an outlet as indicated on the warning label of the appliance by the manufacturer. b) In the Work Area behind the register counter - A Heat Wave heater was plugged into a relocatable power tap instead of an outlet as indicated on the warning label of the appliance by the manufacturer.

On or about January 26, 2023, employees were using extension cords to provide power to equipment including, but not limited to phone chargers.
