29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On and before April 4, 2023, the energy control (Lockout/Tagout) procedure for the CAR Breakdown Press, did not properly outline the steps for the control of the hazardous energy sources that would allow for effective verification. The CAR Breakdown Press procedure incorrectly directs the isolation of the electrical energy first and the pneumatic energy second. Electrical energy is needed to actuate the controls and pneumatic valves of the machine to verify that the pneumatic energy has been isolated and all potentially hazardous stored or residual pneumatic energy is relieved. After verifying the pneumatic energy control measures are effective, then the electrical energy should be isolated, locked out and verified.
