29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm in that employees were exposed to struck-by and crushed-by hazards: a. Warehouse/storage rack near SMP Storage Room: The base plates of the storage rack used to hold pallets of plywood, office supplies, and unlabeled boxes were not anchored to the floor to prevent movement. Employees were exposed to struck-by and crushed-by hazards. b. Warehouse/excess broken equipment storage rack: The base plates of the storage rack used to hold pallets of reams of paper documents were not anchored to the floor to prevent movement. Employees were exposed to struck-by and crushed-by hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Click #2 Go Area: An industrial fan was powered by a relocatable power tap that was series connected "daisy chained" with another relocatable power tap. Employees were exposed to fire and electrical hazards. The Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 1398739, citation number 2, item number 2 and was affirmed as a final order on 11/12/2019, with respect to a workplace located at Bldg 6088 Mokapu Road, MCBH Kaneohe Bay HI 96863.
