29 CFR 1926.451(e)(1): When scaffold platforms were more than 2-feet (0.6m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used:  (a)	This violation occurred on or about January 27, 2023, at a job site located at 6038 Determine Lane, Forney, TX; where employees working from a two-tier scaffold system doing cornice work on a single-family house and did not have a ladder for safe means of access, were exposed to fall hazards.

29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:        (a)	This violation occurred on or about January 27, 2023, at a jobsite located at 6038 Determine Lane, Forney, TX, where employees doing cornice work on a single-family house using a nail gun and were exposed to eye injuries by not wearing eye protection:  ALVARO LOZANO WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 1926.102 (a)(1) OR ITS EQUIVALENT WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1595570, CITATION NUMBER 1, ITEM 1A THAT WAS ISSUED ON JUNE 23, 2022, WITH RESPECT TO A WORKPLACE LOCATED AT 211 OLMSTEAD DRIVE, PRINCETON, TX.   THE FINAL ORDER DATE WAS JULY 22, 2022.
