29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) 579 S Fillmore Road, State College PA, Roof, Fall to Exterior - Employees performed residential roofing activities on a two-story residential roof,  approximately 20 feet above grade with an 6/12 roof pitch, without utilizing any means of fall protection, on or about February 1, 2023.  (b) 579 S Fillmore Road, State College PA, Roof, Fall to the Interior - Employees performed residential roofing activities on a two-story residential roof,  approximately 20 feet above grade with an 6/12 roof pitch, without utilizing any means of fall protection, on or about February 1, 2023.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR  1926.95(a):Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.  (a) 579 S Fillmore Road, State College PA, Roof, - Employees did not don safety glasses while using pneumatic nail guns to attach shingles to the roof of a townhome building and were exposed to flying debris and ricochet nails, on or about  February  1, 2023.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
