29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage firefighting: (a) Green Waste Area - On or about February 1, 2023, the employer did not effectively train employees on portable fire extinguisher use thereby exposing employees to fire and related hazards. An employee was injured while using portable extinguishers to put out fires that resulted from an explosion. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Note: Temporary electrical power and lighting installation(s) may only be used as specified by 1910.305(a)(2)(i)(A) through 1910.305(a)(2)(i)(C). (a) Green Waste Area - On or about February 1, 2023, the employer permitted the use of temporary string lights as a substitute for permanent fixed lighting within the CONEX intermodal shipping container where flammable vapors accumulated. The string light power plug was removed, and the string light was spliced into a wall mounted outlet box where it could be operated by light switch thereby exposing employees to shock and fire hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Where the flexible power cord entered a wall mounted outlet box, strain relief, fittings, or other protections against abrasion were not implemented, thereby exposing employees to shock hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.307(b): Areas designated as hazardous (classified) locations under the Class and Zone system and areas designated under the Class and Division system established after August 13, 2007 were not properly documented: (a) Green Waste Area - On or about February 1, 2023, the employer failed to classify a 40'x8'x8' intermodal shipping container (CONEX) as a hazardous location thereby exposing employees to explosion and fire hazards. The employer used the CONEX as enclosed, short-term storage for plant waste that had been saturated in butane. A spark was generated when an employee attempted to attach a portable space heater to an extension cord attached to an electrical outlet box mounted just inside the CONEX entrance. The electrical equipment provided was not rated for hazardous (classified) locations. The spark ignited flammable vapors and resulted in an explosion and fires within the CONEX. The CONEX and adjacent building were heavily damaged in the blast and the employee was hospitalized with burns to the hands and face. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
