The employer did not ensure an inspection of energy control procedures was conducted for equipment including, but not limited to,  the Spot-welder #30, Spot-welder #33, and spot welder #36. Hazardous energies present on the equipment include but are not limited to pneumatic and electrical.

On or about February 01, 2023, the employer did not ensure periodic inspections of energy control procedures included a review of each authorized employee's responsibilities under the energy control procedures.  Authorized employees are required to conduct service and maintenance activities on the on equipment including but not limited to Spot-welder #30, Spot-welder #33, and spot welder #36. requiring them to perform lockout tagout. Hazardous energies present on the equipment include but are not limited to pneumatic and electrical.

The employer engages in the service and maintenance activities on machinery and equipment including, but not limited to; spot-welder # 30, spot-welder #33, and spot-welder #36.

Employees were operating the resistance welders with the use of a face shield or googles.
