29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: On or about October 12, 2022, the employer failed to record a work-related injury that resulted in restricted duty and days away from work when an employee's finger was caught in a resistance welder, and the employee suffered pulverized bone and a laceration that ultimately resulted in a fingertip amputation. The employer was not keeping OSHA logs up to date on an annual basis.
