29 CFR 1910.147(c)(7)(iii)(A): The employer did not provide retraining for all authorized and affected employees whenever there is a change in their job assignments, a change in machines, equipment or processes that present a new hazard, or when there is a change in the energy control procedures: (a) On or before 1/30/23, the employer did not ensure that authorized and affected employees were retrained whenever there was a change in their job assignments, a change in machines, equipment or processes that present a new hazard, or when there is a change in the energy control procedures. An employee, who was new to his assignment, began work on the Hyster forklift mast without performing proper lockout and tagout of the equipment. These conditions exposed employees to amputation and crushing hazards.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) On or before 1/30/23, the employer did not ensure that employees isolated and controlled the potential energy of the Hyster forklift mast while a hydraulic line holding the mast up was being removed. The employer did not ensure that the hydraulic fluid pressure was de-energized prior to having a hydraulic hose removed. These conditions exposed employees to amputation and crushing hazards.
